This privacy policy explains how Pellizzato Physiotherapy handles the personal information you submit through the contact form on this website. It is written to comply with the EU General Data Protection Regulation (GDPR) and the Dutch implementation of that regulation (Uitvoeringswet AVG).

What information this website collects

This website collects only the information you choose to provide through the contact form, which is:

  • Your name
  • Your email address
  • The content of the message you write

That is the entire scope of personal data collected by the website itself. No other identifiers, account details, payment information, location data, or health information are collected through the site.

Why this information is collected

Your name, email address, and message are used for one purpose only: to read your enquiry and reply to it. This typically means answering a question, providing information about treatments and fees, or arranging a first appointment.

The legal basis for processing this information under GDPR is your consent (Article 6(1)(a)) — by completing and submitting the contact form, you actively choose to share this information so I can get back to you. In some cases the legal basis may also be the steps taken at your request prior to entering into a contract (Article 6(1)(b)), for example when you ask to schedule an appointment.

How long the information is kept

Contact form messages, together with the name and email address attached to them, are kept only as long as needed to complete the conversation and any directly related follow-up. As a general rule:

  • If your enquiry does not lead to an appointment, your message and contact details are deleted within 6 months.
  • If your enquiry leads to an appointment, your contact details may be carried over into the practice's separate patient record system (see "What this policy does not cover" below), which has its own retention requirements under Dutch medical law.

You can ask for your data to be deleted earlier at any time (see "Your rights").

Who has access to your information

The information you submit through the contact form is read only by Beatrice Pellizzato.

It is not sold, traded, rented, or shared with marketing companies, advertisers, or any third party for commercial purposes.

The following service providers process limited data on my behalf strictly to make the website function:

  • The website hosting provider, which technically transmits and stores form submissions.
  • The email provider used to receive and respond to enquiries.

These providers act as "data processors" under GDPR and are bound by their own data protection obligations. They are not permitted to use your information for any purpose other than delivering the service.

Your information may also be disclosed if required by Dutch law, for example in response to a valid order from a competent authority.

Where your information is stored

Data is stored on servers located within the European Economic Area (EEA) wherever possible. If a service provider stores or processes data outside the EEA, this only happens under safeguards permitted by GDPR (such as the European Commission's Standard Contractual Clauses).

Cookies and tracking

This website does not use tracking cookies, advertising cookies, or third-party analytics that profile visitors. If strictly necessary functional cookies are used (for example, to keep the contact form working), they do not identify you personally and are not used for any marketing purpose.

If this changes in the future — for example, if analytics are added to better understand which pages are visited — this policy will be updated and, where required by Dutch law, a cookie banner will ask for your consent before any non-essential cookies are placed.

Your rights

Under GDPR you have the following rights regarding your personal data:

  • Right of access — you can ask what information is held about you.
  • Right to rectification — you can ask for inaccurate information to be corrected.
  • Right to erasure — you can ask for your information to be deleted ("right to be forgotten").
  • Right to restriction — you can ask for processing to be paused.
  • Right to object — you can object to the processing of your data.
  • Right to data portability — you can ask for a copy of your data in a commonly used format.
  • Right to withdraw consent — where processing is based on consent, you can withdraw it at any time. This does not affect the lawfulness of processing carried out before withdrawal.

If you believe your data has not been handled correctly, you also have the right to lodge a complaint with the Dutch Data Protection Authority, the Autoriteit Persoonsgegevens (autoriteitpersoonsgegevens.nl).

Security

Reasonable technical and organisational measures are in place to protect the information submitted through this website, including secure (HTTPS) transmission of form submissions and access controls on the email account that receives them. No method of transmission over the internet is fully secure, but every reasonable effort is made to safeguard your information.

What this policy does not cover

This policy covers only the data collected through this website's contact form. A few related things are governed separately:

  • Patient medical records. Once you become a patient, your health information is held in a separate, secure patient record system (mijnzorgtoegang.nl) and is governed by Dutch medical law, including the WGBO (Wet op de Geneeskundige Behandelingsovereenkomst) and professional confidentiality obligations of BIG-registered practitioners. Different retention periods and legal bases apply to that information.
  • Direct contact by email, phone, or WhatsApp. If you reach out through those channels rather than the website form, the data protection terms of the email provider, telephone carrier, and WhatsApp also apply alongside this policy.
  • External links. The website may link to external services such as the online booking system or insurer pages. Those services have their own privacy policies, which you should review separately.

Changes to this policy

This policy may be updated from time to time, for example if the website changes or the law changes. The "Last updated" date at the top will always reflect the most recent version. Substantive changes will be communicated clearly on the website.
